Non-compliance maxillary molar distalization is an increasingly popular option in resolution of Class II malocclusions. Among the intraoral distalization appliances, distal jet is used widely because of many advantages such as aesthetic, noncompliance, unilateral and bilateral application. The purpose of this study is to compare anchorage loss in unilateral and bilateral distal jet appliance. Patients with skeletal Class I, dental Class II malocclusion and normal vertical development were included. Distal jet appliance was applied bilaterally to 11.1 and 12.6 years old patients and unilaterally to 13.5 years old patient. Class I dental relation, increase in overjet, decrease in overbite, increase in maxillary incisors inclination and upper lip protrusion was occured at the end of the distalization. Despite more teeth support, application of unilateral distal jet was also signified similar anchorage loss with bilateral distal jet. As all subjects were Class II division II, side effects of appliance as protrusion of maxillary incisors and increase in overjet were seen favourable. While using distal jet appliance in molar distalization, initial positions of maxillary incisors must be considered in terms of loss of anchorage. Consequently, if minimal anchorage loss could be disregarded clinically both bilateral and unilateral distal jet appliance could be used efficiently.
