Sarcoidosis involves the lungs in 90% of the cases, but its presentation with a solitary mass lesion is rare. The most common radiologic finding is bilateral hilar or mediastinal lymphadenopathy with or without calcification. A 65 years old female patient presented with a solitary mass lesion invading the aorta and mediastinal lymphadenopathies in magnetic resonance imaging, and malignancy was considered as the diagnosis. However, mediastinoscopy and explorative thoracotomy findings revealed sarcoidosis. We presented the case as it is a rare and a radiologically interesting entity.
